Hi Guys
My sensor was off line (Smartthings) so I force excluded it in the app. I cant add it again
What did I screw up

Couple things to try, because it sounds like the sensor still thinks it is paired with the controller:
Try to Remove it using the Almond LCD. Even though it is not paired some sensors/devices will recognize the remove command anyways and you should see an unknown sensor removed message at the LCD.
Reset the sensor to factory defaults. According to Google that would take:
Make sure the Sensor is powered.
Press and hold the B-button.
Wait for 4-6 seconds for the visual LED indicator to glow yellow (2nd position of the MENU)
Release the B-button.
Click the B-button, once.
Successful reset will be confirmed with the visual indicator changing colour to red and fading.

Thanks that worked.
I still have a problem. I have add it back to ST and for the first few times it senses motion and reports to ST but after that it stops. Is there any way to figure out if its the actual sensor or ST
